DE Policy: Individual Support Planning

NSW Disability Services Standard 2: Individual Needs requires that "Each person with a disability receives a service which is designed to meet in the least restrictive way, his or her individual needs and personal goals".

Disability Enterprises (DE) is committed to providing a service that supports the individual needs and desires of each client. To do this, we use the process of Individual Support Planning (ISP).

An Individual Support Plan is a written plan of goals and strategies for a client, which serves as an action plan for that person and the people supporting him or her. An Individual Support Plan underpins all the services and support that are required to ensure the well-being and safety of the client.

The process used for Individual Support Planning within DE is Person-Centered Planning. This brings together the most important people in the life of a person to help plan lifestyles based on the person's interests, abilities and preferences. This plan maps what a client would like to gain and how he or she would like to achieve it, and what supports DE will will provide.
